Assists clients with a serious and persistent mental illness or a substance use disorder in establishing and
maintaining eligibility for financial and medical assistance, and educates clients regarding entitlements. Assists clients in linking to other needed community services including searching for safe affordable housing, legal services, selecting health care providers, etc. Ensures agency payment for services including establishing funding at onset of services, developing sliding scale or repayment agreements with client, and following-up to minimize uncollected debt.
High School Diploma or equivalent and 4 years paid human service experience, OR Bachelorís degree in
human service related field and 2 years paid human service experience. Experience working with public funding sources strongly preferred. NYS motor vehicle license, safe driving record and availability of personal vehicle for work required.
Full-time, 35 hours.
Monday-Friday, 8:30 AM Ė 4:30 PM. Must be flexible to meet client and program needs.

Hourly salary varies based on education and experience.
