This position is responsible for providing direct care management services to Medicaid eligible individuals and for coordinating all aspects of patient care with outside providers of primary care and community support services. The incumbent will be involved in outreach and engagement activities required to enroll eligible individuals.
1] Locate and engage individuals referred by internal sources at Huther Doyle and Huther Health Clinic
2] Complete a comprehensive Care Management Assessment
3] Develop a patient-centered Care Plan in collaboration with the patient
4] Coordinate patient care with support/health/mental health care services
5] Maintain patient records in accordance with Health Home and Department of Health requirements
6] Make appropriate referrals and provide follow-up
7] Conduct post-treatment follow-up
8] Perform other tasks as assigned
Supervising Care Manager/Director of Health Homes
A BA/BS degree in psychology, counseling, nursing or a closely related field is preferred. At least three year's practical experience as a case manager is required, and experience in a chemical dependency treatment setting would be helpful. Three year's case management experience may be substituted for a degree. Candidate must demonstrate a commitment to working as part of a multi-cultural staff to provide culturally appropriate services to a widely diverse client population.
